Step 1: Fill Out the Application Form
Fill in all required fields, as well as all other applicable fields with accurate information.
For help viewing fields that can be filled in, click Highlight Fields at the top of the form in the Adobe® Reader application.
Step 2: Submit the Application
You have three options for submitting your Driver Application: Email, Fax and Postal Mail. These options are described below.
Submit by Email
On Page 10 of your completed application form, click Submit by Email. This will automatically attach the completed form to an email message addressed to mike.smith@smithtruckinginc.com.
Note: an internet connection is required to submit your form by Email.
Submit by Fax or Postal Mail
On Page 10 of your completed application form, click Print Form. This will send the completed form to your printer, which you can then send your printed application to Smith Trucking, Inc. using the information provided below.
You may also use this to print a copy of this application for your records.
