Thank you for your interest in Smith Trucking, Inc.! We have provided an electronic application form for you to download, fill out, and then submit to us, as per the following instructions.
Step 1: Download the Application Form

Step 1: Fill Out the Application Form
Fill in all required fields.
After opening the application form in Adobe Reader, click on the Highlight Existing Fields box located at the top of the form in the Adobe® Reader application to view all required fields.
Step 2: Submit the Application
To Submit by Email:
At the bottom of Page 11 of your completed application form, click Submit by Email. This will automatically attach the completed form to an email message addressed to mike.smith@smithtruckinginc.com
NOTE: Microsoft Outlook is required to use this option, along with an internet connection.
OR you may save the application as a PDF file and send it as an attachment with the email program you currently use to mike.smith@smithtruckinginc.com
To Submit by Fax or Postal Mail:
At the bottom of Page 11 of your completed application form, click Print Form. This will send the completed form to your printer, which you can then send to Smith Trucking, Inc. using the information provided below.
